Community health nurses can apply the strategies of cultural competence by being culturally aware. Nurses should start by assessing the patient to determine the person’s knowledge and beliefs before providing care. This will give the nurse insight as to how to proceed with education and treatment.

     An example of a nurse practicing cultural preservation would to acknowledge the use of traditional healers within the Hispanic community. Latinos term illnesses as either “hot” or “cold” and treat such illnesses with herbs or other measures, such as massage on the affected area or steam baths. The treatment of such illnesses is termed cultural accommodation and should be encouraged if these forms of treatment have been proven harmless. An example of cultural repatterning would be for the nurse to work with the patient to change a person’s behavior(s) that would be harmful to him/her. Cultural brokering is recognized as connecting differing cultural backgrounds so that a change in behavior can be made to produce a successful outcome. Barriers to applying any of these strategies is a lack of understanding on the nurse’s part and recognizing that family plays a major role in influencing behavior. For example, research shows that Latino adolescents that battle substance abuse are more successful with their treatment and have better outcomes with family support. This is due to, in part, because Hispanic families rely on the entire family (aunts, uncles, cousins, grandparents, etc.) as a unified support system (Burrow-Sanchez). However, some barriers to Latinos obtaining treatment could be a lack of access to needed healthcare/treatment, language barriers, etc.

     Therefore, it is very important for nurses to be culturally aware and practice patience and understanding while maintaining respect when it comes to providing care for people that come from different cultures.

A community health nurse has a great challenge in applying cultural competence because they see and treat such a variety of culturally different people. It is key for the community health nurse to assess their own beliefs and be aware that they may greatly vary from the people in their care. Being open minded and setting aside personal biases while implementing the four strategies of cultural competence is needed to achieve success (Maurer & Smith, 2013).

Midwives work in facilities, but also work in the community as well assisting with birthing in homes. Cultural preservation can be seen by only using a female midwife in the home birth of a Muslim baby as well as respecting that the father will most likely not be present for the birth. Cultural accommodation example would be to allow a mother soon after giving birth to eat a ratb, which is a type of date. This is thought to help promote excellent breast milk for the baby. Cultural repatterning would be discussing that the tradition of placing khakhe shafaa, a type of mud, in the infant’s mouth after birth as unhealthy. Cultural brokering involves another person of the persons culture to help negotiate between the community health nurse and the patient. In the case of the khakhe shafaa, there is an additional practice of using Euphrates water in the baby’s mouth. Using someone based in the same culture will increase the trust level and possible help repattern the behavior that is unhealthy for the infant (Ahlul Bayt Digital Islamic Library Project [Islamic Library], 2018).

Another example of cultural accommodation would be to show support of fasting during Ramadan. Unfortunately, if the Muslim client is diabetic and being seen in a community setting, the nurse may recommend monitoring blood glucose levels and “breaking the fast” if the blood sugar levels were to drop too low. This may create a barrier for applying this strategy (Rassool, 2015).

Urinary tract infections commonly abbreviated as U.T.I are infections that are known to attack the urinary tract. U.T.I can attack either the lower urinary tract or the upper urinary tract or both. Attack of the lower urinary tract is always referred to as bladder infection whereas lower urinary track is always referred to as kidney infection.(Bonadio & Maida, 2014).

According to Tängdén and Giske, (2015), use of single drug to treat U.T.I can be disadvantageous in that single drug use can promote development of dangerous antibiotic resistant bacteria. For instance the single drug used might not be that effective in all parts of the infected urinary parts hence instead of killing the bacteria, they make them bacteria stronger.

Single drug prescription can also be disadvantageous in treating Urinary tract infection in that there might be more than one bacterial infection in the same urinary track. The single drug would only kill the bacteria it’s meant for and not kill other bacteria it’s not meant to eliminate. The results of such scenarios are further development of bacteria on the urinary track that would only threaten the patient’s life. (Brust, Evans, & Plemmons, 2014).

On the other hand single drug prescription can be advantageous in treating of urinary track infection. According to Huttner et. al, (2015).Most single drugs meant for urinary tract infection can treat several bacteria. For instance nitrofurantoin drug has the strength of acting on multiple sites and levels including inhibition of several bacterial enzymes on the urinary track. Single drug prescription for treatment of urinary tract infection is also advantageous in that there’s reduced severity of side effects of single drugs as compared to multiple drug prescription. When side effects become severe, the doctor has the responsibility of prescribing another drug with less severe side effects. This can only be achieved when a single drug is first prescribed and results on the patient determine the next t course of prescription.

       Nephrolithiasis is also known as kidney stones. This is typically a hard deposit of material that forms insides the kidneys. The deposits are made of minerals and salts and they can affect any part of the urinary tract. It is common for the stones to form when the urine becomes concentrated which helps make it easier for the minerals to crystallize and stick together forming the stones. Passing these stones is known to be very painful (Mayo Clinic, 2018). 

       More than 5% of adults are affected by kidney stones in the US and most of these stones are calcium with an association to metabolic disorders. Some risk factors for getting kidney stones could be related to a genetic tendency, obesity, and diet (Worcester & Coe, 2009). The change in diet is believed to be a major factor in the rise of kidney stone occurrence. There appears to be a proportional relationship between the increase in kidney stones and the rate of obesity. The obesity increase is thought to be related to an increase in the consumption of starchy foods and corn. The growing consumption of fast-food and high fructose corn syrup is also related to this change. Other changes to diet include a decrease in the amount of fluid that is consumed (Romero, Akpinar, & Assimos, 2010). Having a digestive disease and surgery like a gastric bypass can also contribute to kidney stones (Mayo Clinic, 2018). Knowing the kind of diet that is commonly found in the US, it is no surprise that the US is affected by kidney stones and that this prevalence is increasing.

Mr. Edwards decides to hire Wendy Jones as the chief operating officer and empower her to cut costs for the hospital. The first measure taken by Ms. Jones is to outsource the interpretation of imaging readings and fire the radiologist Dr. Harris. By doing so, the hospital would save $160,000 per year. However, after two weeks, the hospital sees 18% inaccuracy rates in outsourced reading reports. The hospital might face legal liabilities from inaccurate readings. Mr. Edwards and Ms. Jones want to stay with the plan to control costs, but the physicians are furious. The medical director, Dr. Wiseman, gathers all staff physicians to a call for action.
